When in comes to your health information, you have certain rights under the federal Health Insurance Portability and Accountability Act, often referred to as HIPAA or “HIP-uh.” This law sets rules so that your health information can be shared with the health providers and insurers who have a legitimate need to see it, to
ensure that you receive proper healthcare, while restricting its access by others. Healthcare providers may occasionally ask you to review and sign a copy of the privacy rights form to make sure you understand the rules. You can read a summary of the law here.
United Hospital System has had practices in place for many years to respect patients’ right to privacy and is committed to protecting the confidentiality of protected health information. The Medical Records Department maintains records for patients receiving care at St. Catherine’s Medical Center and Kenosha Medical Center and from United Medical Group physician clinics. The department is located at the Kenosha Medical Center campus and is open 8 a.m. to 4:30 p.m.,
Monday through Friday.
You may request your medical records by filling out an authorization form allowing us to release your private health information. By law, we cannot release medical information requested by telephone, email or through the use of an online request form. Requests for records can only be submitted in person
with proof of identity or by mailing a notarized copy of the completed authorization form to:

Due to the large volume of requests, record copies are not immediately available. Once the Medical Records Department receives your authorization, it will take approximately 10 business days for the record to be photocopied and mailed to the address you provide. If you wish to pick up the
records in person, please inform the Medical Records Department when you request the copies, and bring your photo ID with you when you come to pick them up. If you have questions, please call 262-656-2289.
